**About the Role**
As part of this role you would be responsible for:

- Using a wellness and reablement approach, completing and documenting home support assessments, support plan reviews, linking support and development of support plans for older people and their carers
- Assessing eligibility for the Commonwealth Home Support Program (CHSP) and as required for the HACC Program for Younger People
- Liaising with and referring to internal and external services, maintaining up to date knowledge of services and supports for older people and their carers

**Skills & Experience**

**Essential**
- A tertiary qualification in nursing, allied health, human services or related discipline.
- Demonstrated knowledge of and experience in person centred, strength-based assessment.
- Sound knowledge of and skills in developing innovative and goal directed support plans using a wellness and reablement approach.
- Previous experience working in community assessment and/or community care services.
- Demonstrated experience providing support for persons with vulnerable profiles.
- Demonstrated capacity to actively contribute to continuous quality improvement in response to organisational change, changes in policy contexts, consumer feedback or other practice related issues.
- Excellent Communication Skills - demonstrated ability to engage and communicate with a range of audiences and for a range of purposes, verbally and in writing.
- Current Victorian driver’s licence and Working with Children Card.
