We are seeking an enthusiastic, self-motivated and experienced Program Worker to deliver a range of innovative programs for older people that are aligned with aged friendly, wellness and reablement principles. The Social Support Program Worker operates within a team environment that recognises the contribution and empowerment that comes from older people, family members and carers shaping and influencing the activities they choose to engage in.

**About the Role**
The Social Support Program Worker is responsible for actively delivering social support activities through a range of centre-based, community and online activities designed to suit the individual. Programs are created around the interests of participants with a strong emphasis on co-design and person-centred practices.
Responsibilities include:
- Supporting the engagement of social support activities, working in a structured environment that may be centre or community based.
- Working with people from diverse backgrounds including those with memory loss, disability, cultural diversity and social isolation, and providing assistance with mobility or personal care to clients when needed
**Experience**:
- Certificate III or IV in relevant Aged Care or Disability qualifications
- Manual Handling Certificate with relevance to the aged care sector (desirable)
- Level 2 First Aid (desirable)
- Food Safety Certificate (Level 1 - Basic Food Handling) or ability to successfully complete this certificate.
- A current Victorian Driver’s Licence is an essential requirement and willingness to drive clients in a 12-seater minibus. Access to a roadworthy registered vehicle for work purposes is desirable and Comprehensive Insurance is strongly recommended
