We're looking for a Service Coordinator to support Avivo’s Community Teams in the Western Beaches area. This role will be based out of our Osborne Park office with flexibility to work in the office and from home

The Service Coordinator is a hands-on leadership role that partners with customers to design, develop and coordinate supports and services. They work with customers, colleagues, and Avivo Teams ensuring that customers know their support team and that teams are resourced and developed to provide consistent, quality supports and experiences that go beyond customer expectations.

**Duties include**:

- Leadership, recruitment and development of the team of field based staff
- Planning and providing services against budget within their geographical area
- Working within a Person Centred frame work to ensure support is provided in the most efficient way possible by working closely with the Mentors, Team Coaches Service Coordinator Team, Resource Team, Team Facilitators and Support Workers.
- Have an understanding of individual service design and how to develop supports and services in partnership with customers, carers, and family members; this includes costings, budget development and service agreements.
- Able to develop flexible, innovative practices and living arrangements such as Shared Living with support from internal experts.
- Proactive in seeking opportunities for connection and collaboration with partners including other organisations and community groups to support the inclusion of people we support in their communities.

**Selection criteria**:

- Demonstrates ability to work as a supportive leader, with ability to coach and resource others’ development. Previous experience in supporting and developing employees and teams.
- Knowledge and understanding of disability, aged care and/or mental health sectors and demonstrates contemporary professional knowledge, values and practice.
- Commitment and skills in providing excellent support to customers.
- Understanding and commitment to working with communities to welcome and include all people as valued citizens.
- Excellent interpersonal and communication skills, numeracy and literacy.
- Strong IT skills including ability to use portals, systems and document relevant information.
- Tertiary qualifications preferred but minimum of Certificate 4 in Disability, Aged Care, Mental Health.
