Role Description:

We are seeking a dedicated and compassionate individual to join our team as a Community Connector. This exciting opportunity will allow you to make a real difference in the lives of senior Australians, helping them navigate the aged care system and access the support they need.

The successful candidate will work closely with clients in their community, understanding their needs and connecting them with relevant services. With extensive knowledge of the aged care sector, including the My Aged Care portal, you will be able to identify and coordinate the necessary supports for your clients.

In addition to providing exceptional client care, you will also be responsible for building relationships with key agencies and referral services to ensure seamless service delivery. Your ability to communicate effectively and support diverse backgrounds and life experiences is essential in this role.

Required Skills and Qualifications:
  • Relevant qualifications and/or experience in the aged care sector
  • Local community connections with the target population or a specific sub-group within the population
  • Detailed understanding/ability to rapidly attain a detailed understanding of the range of aged care supports and services
  • Commitment to delivering a person-centred approach that respects and responds to each person's individual needs, preferences, values and life experiences
  • Ability to communicate effectively and support the needs of people with diverse backgrounds and life experience, including Aboriginal and Torres Strait Islander, Culturally and Linguistically Diverse (CALD), and LGBTQIA+ communities
