
Community Connector/Group Planner

**About the Role**:
Do you love making a difference, working in a team environment and creating fun social experiences that connect people living in residential care homes together?
Join ACH Group's Social Programs at West Park located in Goolwa on a part-time basis, working:
- Monday and Tuesday, 9am to 4.30pm
- Wednesday and Thursday, 9am to 4pm
The Group Planning part of the roles will see you manage the logistics and facilitate new and existing groups to maximise customer participation. You will also support the effective delivery of social experiences to enable people to live Good Lives by connecting with their community through experiences that match their interests.
**What you'll do**:
- Deliver social experiences in line with Good Lives (roles, real experiences, right relationships, reablement) and product specifications.
- Work within budgets.
- Assist with documentation and managing quality compliance
- Work with contractors, volunteers and students onsite providing support in social programs
- Lead and run groups and support positive group dynamics and customer engagement
- Lead and run groups
- Assist in organising and running of events

- Capacity to work flexibly in partnership with customers and others using a strong customer service approach.

- Ability to coordination of small to medium sized events.
- Experience in leading and facilitating groups and/or events and supporting participation of group members
- Capacity to work flexibly in partnership with customers and others using a strong customer service approach.
- Computer based documentation to meet accreditation standards.
- Experience working with people at risk of social isolation and loneliness including those from a CALD background and people living with memory loss or Dementia.
- Willingness to travel between residential care homes to backfill as part of the social team.
**You will need**:
- Certificate IV Community Services (Leisure and Health) or equivalent relevant qualification/experience
- A satisfactory National Criminal History Record Check that complies with ACH Group Policy no older than 12 months at commencement of employment.
- A current driver's license with access to a registered and reliable vehicle
- Current vaccinations for Flu and COVID-19
**It would be helpful if you had**:
- Experience working with people at risk of social isolation and loneliness including those from a CALD/ ATSI background and people living with memory loss or Dementia
