Senior Case Manager

We are seeking an experienced Case Manager to join our team. As a Case Manager, you will be responsible for undertaking new consumer assessments and setting up consumers in the care management system.
You will also be required to be responsive to changes in consumer's needs and carry out reviews and reassessments on a regular basis. The role requires strong communication skills, experience working independently, and knowledge of the Community/Aged Care system and home care packages.
Key Responsibilities:- Conduct comprehensive assessments of consumers' needs and develop personalized care plans.
- Implement and review care plans to ensure they meet consumers' changing needs.
- Build and maintain relationships with consumers, their families, and other stakeholders.
- Maintain accurate and up-to-date records of consumer interactions and care plans.
To be successful in this role, you will need:
- Demonstrated case management experience and/or knowledge of the Community/Aged Care system and home care packages.
- A registered nurse qualification (preferable).
- A national police check.
- A driver's licence and a reliable, registered vehicle with comprehensive car insurance.
