Job Title:
Behaviour Support Practitioner
About the Role:
We are seeking a Behaviour Support Practitioner to join our professional and friendly team. As a Behaviour Support Practitioner, you will support clients with behaviours of concern and their families/carers to identify needs through assessment and person-centred planning.
You will work closely with multidisciplinary teams to develop, deliver and evaluate evidence-based support plans within a complex support space. Your goal is to ensure individual plans reflect clients' goals, passions and strengths.
Key Responsibilities:
- Support clients (NDIS participants) and their families/carers to identify needs through assessment and person-centred planning

Requirements:
To be successful in this role, you will need to have:
- Degree level qualifications relevant to providing supports to people with behaviours of concern
- Suitability with the NDIS Commission as a Behaviour Support Practitioner
- Ability to work positively in a multidisciplinary team
- Excellent communication, influencing and interpersonal skills
- Flawless writing skills
- Current Driver's Licence and own reliable vehicle
