Job Title: Specialist Support Coordinator

Job Description:

We are seeking a highly skilled and experienced support coordinator to work with individuals who have complex needs, including psychosocial disabilities, trauma backgrounds, behaviours of concern, or multi-agency involvement.

The successful candidate will be confident in handling high-risk situations, face-to-face interaction, building strong professional networks, and advocating for individuals with multiple systemic barriers.

Key Responsibilities:

  • Delivering specialist support coordination services under the NDIS line item 07_004_0132_8_3
  • Managing complex participant caseloads, including high-risk or crisis situations
  • Coordinating across services: mental health, allied health, housing, safeguarding
  • Assisting with restrictive practice navigation, risk assessments, and planning
  • Leading case conferences and engaging stakeholders in multidisciplinary settings
  • Developing clear, measurable participant goals and progress pathways
  • Maintaining detailed case notes and providing written reports as required by the NDIA

Required Skills and Qualifications:

  • Relevant tertiary qualification in social work, psychology, mental health, community services, or equivalent
  • Proven ability to manage high-risk, high-complexity participants
  • Exceptional communication, stakeholder engagement, and crisis response skills
  • Strong understanding of the NDIS, restrictive practices, and safeguarding
