Individuals with complex disabilities require holistic support to achieve their goals and aspirations. As a Support Coordinator, you will play a pivotal role in empowering participants to exercise choice and control over their services.

Key Responsibilities:
  • Facilitate participant-centered service provision, ensuring that their needs are met through the coordination of multiple stakeholders.
  • Develop and maintain strong relationships with formal mainstream, community, and informal supports to enhance the quality of care.
  • Adhere to the United Nations Conventions on the Rights of People with Disabilities, promoting inclusivity and accessibility in all aspects of service delivery.
  • Manage a complex caseload, prioritizing tasks and ensuring timely completion of services to meet participant expectations.

Day-to-Day Tasks:

  • Implement services in accordance with participant plans, including informal, mainstream, and community supports.
  • Research and coordinate intersecting supports from multiple providers, upholding client choice and control.
  • Identify strategies to manage risks such as sustaining informal supports, housing sustainability, and abuse prevention.
  • Initiate relevant assessments and associated service design to strengthen participant abilities.
  • Empower participants to self-direct their supports and participate in community activities.

This role requires a skilled and independent contractor who can work effectively in a fast-paced environment while maintaining high standards of quality and compassion.
