Support Coordinator Role

We are seeking an experienced professional to fill the position of Support Coordinator.

The successful candidate will be responsible for:

  • Working collaboratively with individuals and their families to maximise their support budget and achieve their goals.
  • Assisting participants to access mainstream services such as housing, education, transport, and health.
  • Building capacity to ensure greater choice and control in directing services and supports.

Key Responsibilities:

  • Attending home and community visits to build relationships and ensure quality and value of received supports.
  • Completing necessary paperwork including case file notes and reporting in a timely manner.
  • Advocating for individuals and their families with dignity and communicating their goals and aspirations.
Requirements
  • Minimum 18 months experience in a similar role.
  • High level of understanding of the NDIS and consumer-directed care principles.
  • Excellent attention to detail and organisational skills.
