The Role

We are inviting a compassionate, resilient and skilled Case Manager to join us. This is a permanent full-time position based in Geelong, VIC.

This role will involve working intensively with a small caseload of individuals experiencing chronic homelessness, delivering outreach-based, long-term case management and service coordination.

You will empower clients to access housing, health care, employment and social inclusion opportunities, and partner with ACCOs and First Nations workers to provide culturally safe support.

Capture meaningful progress through robust outcomes measurement and trauma-informed frameworks.

Key Responsibilities

  • Empower clients to access housing, health care, employment and social inclusion opportunities

Required Skills and Qualifications

A relevant tertiary qualification in Social Work, Psychology, Community Services or related field is essential for this role.

Demonstrated experience working with people with complex needs—homelessness, mental health, substance use, family violence—is also required.

You will need to have a values-driven, culturally aware approach to supporting people from all backgrounds.
