Homelessness Outreach Specialist

We are seeking a skilled and compassionate Homelessness Outreach Specialist to join our team. This is an exciting opportunity for someone who wants to make a meaningful difference in the lives of individuals experiencing chronic homelessness.

About the Role

As a Homelessness Outreach Specialist, you will play a vital role in delivering intensive case management services to individuals with complex needs. You will work closely with clients to empower them to access essential resources such as housing, healthcare, employment, and social inclusion opportunities.

  • You will provide assertive and persistent outreach support to individuals who are homeless or at risk of becoming homeless.
  • You will build deep, trusting relationships with clients and help them to achieve lasting stability and connection.
  • You will partner with community organizations to provide culturally safe support and ensure that clients receive comprehensive services tailored to their individual needs.
  • You will capture meaningful progress through robust outcomes measurement and trauma-informed frameworks.
Your Qualifications and Experience
  • A relevant tertiary qualification in Social Work, Psychology, Community Services or a related field is required.
  • Demonstrated experience working with people with complex needs – homelessness, mental health, substance use, family violence – is essential.
  • Experience in outreach, trauma-informed care, case management, or advocacy is highly desirable.
  • You must have a values-driven, culturally aware approach to supporting people from all backgrounds.
