The Role
The Transitional Support role provides case management support, including outreach, to community members who are homeless and require assistance beyond immediate support received in the IAP space. The position also provides case management to community members nominated to be tenanted in one of the 4 adult or youth Aboriginal-specific THM's under Wathaurong. With a strong focus on strengths-based practice, the case manager works alongside community members to strengthen their capacity to establish and maintain transitional and long-term housing options, while addressing support needs contributing to homelessness. The role also involves supporting and providing outreach to community members in crisis and emergency accommodation.
