We are seeking a skilled Care Coordination Specialist to join our team. In this role, you will work intensively with individuals experiencing chronic homelessness, delivering long-term case management and service coordination.

About the Role:

You will be responsible for building deep, trusting professional relationships through assertive and persistent engagement. This will empower clients to access housing, health care, employment, and social inclusion opportunities.

Key Responsibilities:
  • Deliver outreach-based, long-term case management and service coordination

Requirements:
  • A relevant tertiary qualification in Social Work, Psychology, Community Services or related field
  • Demonstrated experience working with people with complex needs-homelessness, mental health, substance use, family violence
  • Experience in outreach, trauma-informed care, case management, or advocacy
  • A values-driven, culturally aware approach to supporting people from all backgrounds
  • A current Employee Working with Children's Check
  • A current VIC Driver's Licence
