
Homeless Outreach Coordinator

This role is part of our Specialist Homelessness Services team, working in a dynamic and supportive environment with health services, local agencies, and other service providers.
We conduct intake assessments and create individualised, strengths-based case plans. Housing support workers also support clients in accessing safe housing options and making informed decisions, and coordinate referrals and liaising with support services and external agencies.
Key responsibilities include conducting outreach visits and offering hands-on support in residential settings, facilitating life skills and development activities to promote long-term independence, and assisting clients with training, employment, and community engagement pathways.
The ideal candidate will have a social work or relevant tertiary qualification, experience supporting individuals with complex needs and behaviours, and strong relationship-building skills and a collaborative approach.

- Ability to remain calm, compassionate, and solution-focused
- A current Victorian Driver's Licence
- A valid Employee Working with Children Check
