Delivering high-quality care is at the heart of our organization. As a Care Manager, you will play a pivotal role in ensuring that clients receive exceptional support and attention.

Key Responsibilities:
  • Build strong relationships with clients, their families, and representatives by providing empathetic and personalized care.
  • Conduct regular quality assurance visits to monitor care, address changing needs, and recommend additional services as required.
  • Collaborate with clients, families, caregivers, and other stakeholders to ensure care plans remain aligned with evolving needs and preferences.
About You:
  • Tertiary qualification in a health-related field is desirable, such as social work or allied health, with at least 1 year of experience working with Home Care Packages and seniors in aged care.
  • Strong organizational skills with the ability to manage time effectively, multitask, and adapt to shifting priorities under pressure.
  • Excellent written and verbal communication skills, with accurate data entry and proficiency in Microsoft Office.
  • Must hold a valid Australian driver's license and be comfortable traveling as required.
