Job Description

We are seeking an experienced Care Manager to join our team. This full-time role will play a crucial part in supporting elderly and disabled clients to live independently and with dignity.

The successful candidate will develop and implement personalised care plans, coordinate and oversee the delivery of high-quality in-home care and support services, liaise with clients, their families, and healthcare professionals, and monitor client progress.

Key Responsibilities

Requirements

Applicants must have a minimum 1 year's experience in a similar Care Manager or senior care coordination role. A Diploma of Community Services, Nursing, or Allied Health qualification is also required.

Excellent communication and interpersonal skills, with the ability to build strong relationships with clients and their families, are essential.

Strong organisational and problem-solving skills, with a focus on delivering exceptional client outcomes, are also necessary.
