Disability Navigator Role Overview

The Disability Navigator role is a critical position in the healthcare sector, requiring an individual with strong clinical skills and experience working with individuals with disabilities. As a Disability Navigator, you will play a key role in providing care coordination, case management, and navigation for people with disabilities who are hospitalized, ensuring they have appropriate supports for discharge and facilitating a successful transition back into their communities.

Key Responsibilities:
  • Provide care coordination and case management services to individuals with disabilities who are hospitalized
  • Navigate complex healthcare systems to ensure seamless transitions and optimal outcomes
  • Work collaboratively with multidisciplinary teams to develop and implement effective care plans
  • Communicate effectively with patients, families, and caregivers to ensure understanding and engagement in the care planning process
Required Skills and Qualifications:

To be successful in this role, you will need:

  • Tertiary qualification in a relevant field (e.g. social work, allied health)
  • At least 3 years of clinical experience working with individuals with disabilities
  • Strong clinical reasoning and problem-solving skills
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
