Job Opportunity

We are seeking a highly skilled Disability Care Coordinator to join our team.

The successful candidate will be responsible for providing care coordination, case management and navigation for individuals with disabilities who are in hospital. This includes ensuring they have access to appropriate supports for discharge, thereby facilitating a smooth transition back into the community.

Key Responsibilities:
  • Provide holistic care coordination and case management services to individuals with disabilities.
  • Support individuals eligible for NDIS services who are at risk of hospitalisation or require expedited access to these services to alleviate pressure on healthcare services.
  • Collaborate closely with multidisciplinary teams to ensure seamless transitions of patients into community-based care.
Requirements:
  • A tertiary qualification in a relevant field, preferably in social work or an allied health profession, along with 3 years of clinical experience and eligibility for registration with the relevant professional body.
  • Demonstrated advanced clinical reasoning, experience and practice skills relevant to assessment and treatment of individuals with disabilities.
  • Ability to coordinate and communicate complex care plans within a healthcare context as part of a multi-disciplinary team.
  • Capacity to complete reports using language relevant and suitable for government agencies.

This role requires strong communication and interpersonal skills, as well as the ability to work autonomously and collaboratively as part of a team. The successful candidate must also possess a current unrestricted driver's license and be willing to travel across the region.
