Disability Service Coordinator

As a Disability Navigator, you will play a critical role in ensuring the successful discharge of people with disabilities from hospital. You will provide care coordination and navigation to ensure that patients have the necessary support for their transition back into the community.
The ideal candidate will possess advanced clinical reasoning skills, experience, and practice skills relevant to assessment and treatment of people with disabilities. They will also have demonstrated experience working in the disability sector, with a strong understanding of navigating the National Disability Insurance Scheme (NDIS) and alternative disability-related supports.
Key responsibilities include:
- Providing care coordination, case management, and navigation for people with disabilities who are in hospital.
- Supporting NDIS-eligible individuals who are in hospital, at risk of hospitalization, or where the support will expedite access to NDIS services to relieve pressure on healthcare services.
Required Skills and Qualifications
To be successful in this role, you will need to demonstrate:
- Tertiary qualification and 3 years' clinical experience as an Allied Health Clinician preferably in Social Work or a relevant Allied Health profession, with eligibility for registration with the appropriate professional body.
- Demonstrated advanced clinical reasoning, experience, and practice skills as relevant to assessment and treatment of people with disabilities.
- Demonstrated experience working in the disability sector, with a strong understanding of navigating the NDIS and alternative disability-related supports.
