Disability Support Specialist
">The Disability Support Specialist plays a vital role in facilitating the successful transition of individuals with disabilities from hospital care by providing coordinated support, case management, and navigation services.
">Key Responsibilities:
- Provide personalized assistance to individuals with disabilities who are hospitalized, at risk of hospitalization, or require expedited access to disability services to alleviate pressure on healthcare resources.
- Support individuals with disabilities throughout their journey, including assessment, treatment, and ongoing care coordination.
- Collaborate with multidisciplinary teams to develop and communicate comprehensive care plans.
Requirements:
- Relevant tertiary qualification and 3 years of experience as an Allied Health Clinician, preferably in Social Work or a related field, with eligibility for registration with the appropriate professional body.
- Demonstrated advanced clinical expertise, experience, and practice skills relevant to assessing and treating individuals with disabilities.
- Ability to prioritize caseloads, manage stressful situations, and work independently.
- Current unrestricted drivers' license and willingness to travel across designated areas.
