Job Description

The Disability Navigator role involves providing care coordination, case management and navigation for individuals with disabilities in hospital settings. This ensures they receive appropriate support for discharge, leading to successful transitions.

The position requires collaboration with multidisciplinary teams to develop and implement complex care plans. Effective communication and negotiation skills are essential for managing relationships with various stakeholders.

A key aspect of the role is supporting NDIS-eligible individuals who are in hospital, at risk of hospitalization, or require expedited access to services to alleviate pressure on healthcare services.


Required Skills and Qualifications
  • Tertiary qualification in a relevant field (e.g., Social Work, Allied Health) with 3 years' clinical experience and eligibility for registration with the relevant professional body.
  • Demonstrated advanced clinical reasoning, assessment, and treatment skills for individuals with disabilities.
  • Experience working in the disability sector, understanding navigating the NDIS, and alternative support systems.
  • Ability to prioritize caseloads, manage stress, and work autonomously.
  • Effective communication and negotiation skills for interacting with diverse stakeholders.
