Healthcare Navigator Role

This is a rewarding opportunity to make a positive impact on the lives of individuals with disabilities. As a skilled navigator, you will play a vital role in ensuring that people receive the necessary support and care coordination throughout their hospital stay.

Key Responsibilities:
  • Coordinate care and provide case management for individuals with disabilities who are hospitalized, ensuring they have the necessary resources for a smooth discharge.
  • Work collaboratively with the Specialist Intellectual Disability Health Team (SIDHT) Outreach Clinician for the Far West Local Health District, performing duties as required by this team.
  • Support individuals eligible for NDIS services who are hospitalized, at risk of hospitalization, or where support will expedite access to NDIS services, alleviating pressure on local health district services.
