About the Role:

We are seeking a highly skilled and experienced Disability Support Specialist to join our team. The successful candidate will provide care coordination, case management and navigation for people with a disability who are in hospital.

Key Responsibilities:

  • Provide care coordination, case management and navigation for people with a disability who are in hospital to ensure they have appropriate supports for discharge which would ensure a successful discharge from hospital.
  • The Disability Support Specialist is expected to perform outreach duties across the Far West Local Health District, supporting NDIS eligible people who are in hospital, at risk of hospitalisation or where the support will expedite access to NDIS services to relieve pressure on local health district services.
  • Support individuals with complex needs, providing tailored care plans and effective communication with healthcare teams.

Requirements:

  • Tertiary qualification in a relevant field and 3 years clinical experience as an Allied Health Clinician, with eligibility for registration with the appropriate professional body.
  • Demonstrated advanced clinical reasoning, experience and practice skills as relevant to assessment and treatment of people with disability.
  • Demonstrated ability to coordinate and communicate complex care plans within a healthcare context, as part of a multi-disciplinary team.
  • Demonstrated experience of effective negotiation, advocacy, collaboration and communication skills with a wide range of stakeholders.
