Job Title: Preventable Chronic Conditions Specialist
We are seeking a skilled professional to fill the role of Clinical Specialist in Preventable Chronic Conditions. The successful candidate will be responsible for providing specialist clinical outreach care to remote clients and supporting PPHC staff in improving outcomes in early detection and management of preventable chronic conditions.
About the Role:- To assess, plan, review, evaluate, and coordinate comprehensive client care and/or casemanagement
- To deliver expert care and education to improve chronic condition management across remote NT communities
- To collaborate with multidisciplinary teams to develop and implement effective health programs and services
Requirements:
- A current registration with the Nursing and Midwifery Board of Australia or eligibility for same
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills
- A commitment to a learning culture through clinical guidance and mentoring
- The ability to travel to remote communities by 4WD or light aircraft as required
- A current NT driver's licence (or eligible to obtain) and confidence driving a manual vehicle in regional and remote settings
