Nurse/Midwife Specialist in Preventable Chronic Conditions
We are seeking a skilled professional to deliver expert care and education to improve chronic condition management across remote communities.
Job DescriptionThis role involves providing specialist clinical outreach care to remote clients and supporting public health care staff in improving outcomes in early detection and management of preventable chronic conditions.
The successful candidate will be accountable for providing professional advice, assessment, planning, review, evaluation, and coordination of comprehensive client care.
Required Skills and Qualifications- Current registration with the Nursing and Midwifery Board of Australia or eligibility for same.
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
- Commitment to a learning culture through clinical guidance and mentoring.
- Ability to travel to remote communities and hold a current NT driver's licence.
