The role of a Specialist Preventable Chronic Conditions Nurse/Midwife involves delivering expert patient care and education to improve chronic condition management in remote Northern Territory communities.
- Key Responsibilities:
- Deliver specialist clinical outreach services to patients in remote areas, providing support, advice, and education to primary healthcare staff to enhance outcomes for early detection and management of preventable chronic conditions in a culturally sensitive manner.
- Conduct comprehensive client assessments, develop and implement individualized care plans, and coordinate casemanagement as needed.
To succeed in this position, you will require:
- Current registration with the Nursing and Midwifery Board of Australia or eligibility for same.
- Demonstrated high-level clinical knowledge and skills in Preventable Chronic Conditions or a related field, supported by strong decision-making and problem-solving abilities.
- A commitment to ongoing professional development through clinical guidance and mentoring.
- The ability to travel to remote locations using 4WD vehicles or light aircraft as required.
- A valid Northern Territory driver's license (or eligible to obtain) and confidence driving a manual vehicle in regional and remote settings.
