Nurse/Midwife Specialist Preventable Chronic Conditions

Seeking a skilled Nurse/Midwife Specialist to deliver high-quality care and education in the field of preventable chronic conditions.
- The successful candidate will provide specialist clinical outreach services to remote clients and support PPHC staff in improving outcomes for early detection and management of preventable chronic conditions.
This is an excellent opportunity for a motivated and experienced healthcare professional to make a positive impact in the community.
- Key Responsibilities:
- Provide sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or casemanagement.
- Deliver expert care and education to improve chronic condition management across remote NT communities.
- Travel to remote communities by 4 WD or light aircraft as required.
To be successful in this role, you'll need:
- Current registration with the Nursing and Midwifery Board of Australia.
- High-level clinical knowledge and skills in preventable chronic conditions or related field.
- Commitment to a learning culture through clinical guidance and mentoring.
- Ability to travel to remote communities and hold a current NT driver's licence.
