Job Title

Seeking a skilled Nurse/Midwife Specialist to deliver high-quality care and education in the field of preventable chronic conditions.

  • The successful candidate will provide specialist clinical outreach services to remote clients and support PPHC staff in improving outcomes for early detection and management of preventable chronic conditions.

  • Key Responsibilities:
  • Provide sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or casemanagement.
  • Deliver expert care and education to improve chronic condition management across remote NT communities.
  • Travel to remote communities by 4 WD or light aircraft as required.
Requirements

To be successful in this role, you'll need:

  • Current registration with the Nursing and Midwifery Board of Australia.
  • High-level clinical knowledge and skills in preventable chronic conditions or related field.
  • Commitment to a learning culture through clinical guidance and mentoring.
  • Ability to travel to remote communities and hold a current NT driver's licence.
