Deliver expert care and education to improve chronic condition management across remote Northern Territory communities.

Job Summary:

The Nurse Clinical Specialist (Preventable Chronic Conditions Educator) provides specialist clinical outreach care to remote clients and specialist support, advice, and education to Primary Health Care staff with a focus on improving outcomes in early detection and management of preventable chronic conditions in a culturally sensitive manner.

Key Responsibilities:
  • Provide sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.
  • Develop and implement effective strategies for preventing and managing chronic conditions in remote communities.
  • Collaborate with Primary Health Care staff to provide specialist support and education.
Required Skills and Qualifications:
  • Current registration with the Nursing and Midwifery Board of Australia or eligibility for same.
  • High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
