Job Summary:The Nurse Clinical Specialist (Preventable Chronic Conditions Educator) provides specialist clinical outreach care to remote clients and specialist support, advice, and education to Primary Health Care staff with a focus on improving outcomes in early detection and management of preventable chronic conditions in a culturally sensitive manner.
Key Responsibilities:- Provide sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.
- Develop and implement effective strategies for preventing and managing chronic conditions in remote communities.
- Collaborate with Primary Health Care staff to provide specialist support and education.
- Current registration with the Nursing and Midwifery Board of Australia or eligibility for same.
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
