Improve chronic condition management across remote NT communities as a Clinical Specialist.
The Role- Provide specialist clinical outreach care to remote clients and support PPHC staff in improving outcomes in early detection and management of preventable chronic conditions.
- Accountability for providing sound professional advice and assessment, planning, review, evaluation, and coordination of comprehensive client care and/or case management.
- Current registration with Nursing and Midwifery Board of Australia or eligible for same.
- High-level clinical knowledge and skills in Preventable Chronic Conditions or related field, supported by strong decision-making and problem-solving skills.
- Commitment to a learning culture through clinical guidance and mentoring.
- Ability to travel to remote communities by 4WD or light aircraft as required.
