Care Coordinator

We are seeking experienced professionals to join our team as Care Coordinators. This autonomous role offers the opportunity to enhance your clinical skills and provide care coordination, navigation, and health coaching services to individuals with chronic conditions and complex needs.

The ideal candidate will develop individualised care plans, refer clients to other providers as needed, navigate across multi-disciplinary services required to fulfil the care plan, and support self-management principles.

To be successful in this role, you will require:

  • Relevant qualification in Nursing (Registered Nurse) or Allied Health with demonstrated experience in your field of expertise;
  • Experience in delivering chronic condition management, healthcare navigation, health coaching, and the principles of health literacy;
  • Well-developed communication (verbal, written, and listening) and interpersonal skills to liaise with clients, carers, families, and other healthcare professionals;
  • Demonstrated ability to provide education, support, and coaching to clients and their carers;
  • Demonstrated time management and organisational skills to effectively plan and prioritise work;
  • Demonstrated ability to work both autonomously and with a team to participate and contribute to program and organisational objectives;
  • A demonstrated empathetic and flexible approach in all interactions with clients, carers, and other healthcare providers.
