Job Description:
We are seeking a compassionate and skilled professional to join our team as a Homeless Support Specialist. In this role, you will work with individuals who are homeless or at risk of homelessness, providing them with the support they need to reclaim their lives and build towards a brighter future.
About the Role:
This is a fixed-term full-time position that involves working within a dynamic and supportive environment where collaboration is key. You will be part of a specialist homelessness services team that works closely with health services, local agencies, and other service providers to provide holistic care to our clients.
Responsibilities:
Conduct intake assessments and create individualised case plans that cater to the unique needs of each client
Support clients in accessing safe housing options and making informed decisions about their future
Coordinate referrals and liaise with support services and external agencies to ensure seamless delivery of care
Conduct outreach visits and provide hands-on support in residential settings to promote long-term independence
Facilitate life skills and development activities that empower clients to make positive changes in their lives
Assist clients with training, employment, and community engagement pathways to enhance their overall well-being
Qualifications:
A Social Work or relevant tertiary qualification (or significant related experience)
Experience supporting individuals with complex needs and behaviours
Strong relationship-building skills and a collaborative approach
Ability to remain calm, compassionate, and solution-focused during challenging situations
