  • Collaborate with therapists, nurses, and care coordinators to develop and implement tailored support plans.">
  • Assist with daily living activities, including self-care, medication reminders, and mobility support.">
  • Help clients develop coping strategies, build routines, and access community resources.">
  • Monitor client wellbeing, promptly report concerns, and document progress accurately.">
  • Support clients in attending therapy sessions, appointments, and social/community activities.">
  • Foster independence while respecting individual rights, preferences, and cultural values.">
  • Promote continuity of care by liaising with other services, family members, and care teams.">

Requirements:

  • Experience in a support worker or care assistant role (transitional care experience is an asset).">
  • Understanding of recovery-based approaches, trauma-informed care, and safeguarding principles.">
  • Excellent communication, problem-solving, and interpersonal skills.">
  • Ability to work independently and as part of a multidisciplinary team.">
  • Flexibility to work various shifts, including evenings or weekends, if required.">
  • Relevant qualification in Health and Social Care (C4) or equivalent preferred.">
