About this role:

As a Care Facilitator, you will be working in a team to deliver high-quality care and services to our Social Club customers. Your main goal is to provide them with meaningful activities and interactions in a safe environment.

You will develop genuine connections with customers and their loved ones to create programs that promote their health, wellbeing and independence. This includes developing individualised care plans and activities that meet customer needs, goals and preferences.

Your day-to-day tasks may include reviewing and updating care plans, supporting direct reports through well-planned team meetings and regular supervision, and delivering timely, professional responses to customer feedback and complaints.

About you:

We are looking for someone with excellent verbal communication skills and an empathetic, patient and caring nature. You will need to promote a team culture where all staff and customers feel valued and respected.

The ideal candidate will have a Bachelor's Degree in a related field or equivalent qualification, knowledge and experience. They will also have experience in facilitating social support group programs and activities within aged care or similar environments.
