Hearth Support Services is a rapidly growing NDIS Registered Service Provider offering an exciting opportunity for a dedicated Relationship Manager to join our friendly team in Bendigo.

Responsibilities:

  1. Develop and maintain strong relationships between participants and their support worker teams
  2. Assess participant needs and design tailored support programs
  3. Maintain effective relationships with support coordinators, house supervisors, and external stakeholders
  4. Lead support worker teams to deliver person-centered care that enables participants to achieve their goals
  5. Monitor progress and prepare submissions for NDIS plan reviews as required
  6. Coordinate weekly staff rosters for each participant and resolve rostering issues
  7. Accurate case noting and client reviews reflecting forward planning aligned with NDIS plans
  8. Ensure compliance with Hearth policies and procedures to guarantee a safe working environment and timely incident reporting

Requirements:

  • Proven experience in disability services, preferably in supervisory or managerial roles
  • Professional administrative and business communication skills, including case notation and report writing
  • Excellent time management and organisational skills
  • A client-centred approach with the ability to advocate for participants and understanding of complex disabilities and trauma
  • Personal vehicle and driver's license
  • Strong interpersonal skills and high levels of resilience
  • Knowledge of the NDIS and Microsoft Office suite
