About the Role

As a key member of our team at Hearth Support Services, you will play a vital role in fostering relationships between participants and their support workers. Your exceptional communication skills and ability to assess participant needs will be crucial in developing Hearth's support program.

Key Responsibilities
  • Relationship Building: Create and maintain strong relationships between participants and their support teams, ensuring seamless communication and collaboration.
  • Needs Assessment: Conduct thorough assessments of participant needs and develop tailored support programs to meet their requirements.
  • Stakeholder Engagement: Foster positive relationships with support coordinators, house supervisors, and other external stakeholders to ensure effective coordination of care.
  • Leadership: Lead support worker teams to provide responsive, person-centred care that enables participants to achieve their goals.
  • Reporting and Compliance: Accurately report on participant progress and prepare submissions for NDIS plan reviews as required, while maintaining compliance with Hearth policies and procedures.
About You

We are looking for an experienced professional with a proven track record in the disability sector, preferably in a supervisory or managerial role. You will possess excellent administrative and business communication skills, including case noting and report writing. A high level of resilience, strong interpersonal skills, and the ability to advocate for participants are essential.

You will also have a working knowledge of Microsoft Office Suite and employee rostering systems, as well as a valid driver's license and own car.
