Hearth Support Services is a rapidly growing NDIS Registered Service Provider committed to delivering exceptional care and support to individuals with disabilities.
As a central figure in our team, you will be responsible for fostering strong relationships between participants, their family members, and support workers.
About the role:
The Disability Program Manager plays a vital role in ensuring that participants have a clear point of contact and are provided with responsive, person-centered care that enables them to progress toward their goals.

Responsibilities will include:
- Creating and continuously developing relationships between participants and their team of Hearth Support Workers
- Assessing participant needs, and developing Hearth's support program
- Maintaining relationships with support coordinators, house supervisors, and other relevant external stakeholders
- Lead the Support Worker teams to provide responsive, person-centered care that enables participants to progress toward their goals
- Reporting on the progress of clients, as it relates to the support provided by Hearth, and preparing submissions for NDIS plan reviews as required
- Coordinating a weekly staff roster for each participant and proactively resolving rostering issues.
- Accurate case noting and client reviews that reflect forward planning in line with the participant's NDIS plan
- Compliance with Hearth policies and procedures to ensure a safe working environment and detailed, timely incident reporting
Requirements:
- Proven experience working in the disability sector, preferably in a supervisory or managerial role
- A Professional level of administrative and business communication skills, including case noting and report writing
- Excellent time management and organisational skills
- Proven client centred approach with the ability to advocate for Participants and an understanding of complex disability and trauma
- Own car and driver's license
- Strong interpersonal skills and high levels of resilience
- Understanding of the NDIS
- Working knowledge of Microsoft office suite and employee rostering systems
The successful candidate will receive a competitive salary of $80,000 - $90,000 per annum, depending on experience, and enjoy a range of benefits including flexible working hours, travel compensation, work laptop, and mobile phone provided.
Salary: $85,000 per annum
