Job Title: Mental Health Care Coordinator


About the Role

This is a challenging yet rewarding role for individuals who are passionate about delivering high-quality mental health care services. As a Community Transitions Clinician, you will be working with a multidisciplinary team to support people who experience severe mental illness and other related issues. Your primary focus will be on facilitating successful transitions from custody to the community.


Key Responsibilities:

  • Work collaboratively with the Custodial Mental Health Service, primary health networks, Community Mental Health and Drug and Alcohol Services, and other organizations to provide a comprehensive approach to ensuring continuity of care.
  • Utilize general care coordination and discipline-specific skills to link patients to relevant services in the community and support their initial reintegration into community care.

Requirements

To be successful in this role, you will need to have:


  • A Bachelor or postgraduate degree in a relevant allied health profession (e.g., psychology, occupational therapy, or social work).
  • Registration with the relevant professional board (Occupational Therapy Board of Australia/ Psychology Board of Australia) or eligibility for membership with the Australian Association of Social Workers.
  • A minimum of 3 years' experience working in mental health settings.
