Mental Health Coordinator

High-Quality Care Coordinators play a vital role in delivering exceptional care to older adults with complex mental health needs.
In this position, you will work collaboratively with a dynamic multidisciplinary team to provide seamless care coordination across multiple sites.
Key responsibilities include:
- Providing expert guidance and education to internal and external partners, including Residential Aged Care Facility staff
- Conducting comprehensive mental health assessments and developing, implementing, and evaluating consumer-focused comprehensive care plans based on evidence-based practice
- Demonstrated experience working with consumers and their carers presenting with complex mental health needs and/or behaviours of concern
The ideal candidate will have:
- A minimum of 5 years full-time post-graduate experience in a mental health specialty
- An approved post-graduate qualification within the speciality of mental health drug and alcohol or other such qualifications or experience deemed appropriate
