Transition Support Role

The Transition Support role plays a vital part in ensuring a seamless transition for customers preparing to receive a Home Care Package. The successful candidate will be responsible for coordinating services that meet individual needs and goals, making a real difference in the lives of older people.

This is an opportunity for a motivated and caring individual to work collaboratively with external stakeholders, including RAS, MAC, and ACAT. Key responsibilities include:

  • Coordinate services for allocated customer load
  • Meet individual KPIs and contribute to team targets
  • Conduct reviews, develop support plans, refer on for clinical and care interventions, and review customer progress
  • Liaise effectively with external stakeholders
  • Undertake risk assessments of activities and services for individual customers

Requirements:

  • Certificate IV in Community Services/Aged Care/Service Coordination or equivalent experience
  • Demonstrated experience in assessment, planning, provision, and evaluation of services
  • Intermediate/advanced level knowledge of Microsoft Office
  • Excellent documentation and administration skills
  • Enthusiastic team member and problem solver
  • Caring attitude and commitment to supporting older people
