Overview

Connections Transition Coordinator - PFT - Wellington role at NSW Justice Health & Forensic Mental Health Network. Permanent Full Time position located in Wellington/Dubbo. Employment Type: Permanent Full Time. Position Classification: Health Clinician L3 including classifications of Social Worker, Counsellor, Occupational Therapist, Psychologist.

Responsibilities
  • The Connections Program aims to improve continuity of care for patients in public correctional centres with Drug and Alcohol problems who are being released to the community, using an assertive linkage model to connect patients with relevant health and welfare service providers pre- and post-release.
  • The Connections Transition Coordinator will utilise a broad array of contacts in both correctional and community environments to link patients into services appropriate to their individual needs.
Salary & Benefits

Your salary of $112,396 (Year 1) - $116,038 (Year 2) for Social Worker, Counsellor and Occupational Therapist classifications or $77,358 (Year 1) to $113,226 (Year 9) for Psychologists + super and leave loading is just the beginning.
