About the Role

As a Case Manager, you will play a vital role in supporting clients under various government programs. This position requires strong interpersonal skills and the ability to develop personalized care plans that align with each client's needs and aspirations.

Key Responsibilities:

  • Provide information, support, and referrals to clients.
  • Develop and regularly review progress of care plans, adjusting them as needed to ensure clients receive the right services.
  • Manage client budgets effectively and oversee resource allocation.
What We're Looking For:

We are seeking an experienced individual who can provide effective case management services to our clients. Key requirements include:

  • Experience in goal setting and care planning.
  • Formal training in Case Management or willingness to complete relevant training.
  • Previous experience in community care or aged care sector is highly desirable.
  • Strong ability to manage client budgets effectively.
  • Current C or CA Driver's License for Queensland is essential.

The ideal candidate will possess excellent communication skills, be able to navigate regulated environments, and have knowledge of clinical governance and aged care standards.
