Family Support Practitioner

We are seeking a skilled and compassionate Family Support Practitioner to provide support for families in inner and outer metro Melbourne. Our services are delivered in person, over the phone and through telehealth outreach.

As a Family Support Practitioner, you will be part of our team that nurtures and fosters a culture where our practitioners are empowered, supported and safe through the process of caring for children and families. You will have access to education and training to enhance your personal and professional development; expert specialist resources; and a comprehensive supervision model that will support you to make a significant impact in the lives of children and their families.

Our state-of-the-art hospice is a world-class facility to support the complex needs of the children and young people in our care. The ideal candidate will have tertiary qualifications in social work, psychology or related discipline, with demonstrated experience working with children and families in grief and bereavement, family services, or the general health field.

Key Responsibilities:

  • Provide trauma-informed therapeutic intervention, including formal counselling and emotional support to one or more family members, including children and young people.
  • Undertake psychosocial assessment and develop, implement, and review level of support required to meet the changing needs of families.
  • Work independently and as part of a larger team of practitioners, providing flexibility and adaptability.
  • Develop and maintain relationships with families and communities, particularly Aboriginal and Torres Strait Islander, LGBTIQ, refugee and culturally diverse families and communities.

Requirements:

  • Tertiary qualifications in social work, psychology or related discipline.
  • Demonstrated experience working with children and families in grief and bereavement, family services, or the general health field.
  • Ability to communicate effectively and empathetically with families and individuals from diverse backgrounds.
  • Flexibility and adaptability to work in a dynamic environment.
  • Current driver's licence and willingness to travel.
