
Senior Case Management Specialist

Senior Case Managers play a vital role in providing intensive case management support to individuals and families with complex needs.
This is a challenging yet rewarding position that requires strong case management skills, cultural awareness and the ability to work autonomously in a fast-paced environment.
Responsibilities:- Provide comprehensive case management services to clients with complex needs, including trauma, mental health, physical health, child protection and family violence.
- Develop and implement tailored case plans that address the unique needs of each client.
- Build strong relationships with clients, their families and other relevant stakeholders to ensure effective service delivery.
- Contribute to the development and implementation of policies and procedures that promote best practice in case management.
- A recognised degree qualification in Social Work, social sciences, psychology or mental health.
- Proven experience in case management, preferably in a similar context.
- Ability to work autonomously while effectively managing competing priorities in a fast-paced environment.
- Skilled in applying trauma-informed, strengths-based and culturally responsive practice to achieve positive outcomes.
- Monthly rostered day off (RDO).
