Case Management Specialist

We are seeking a Case Management Specialist to join our dedicated team, responsible for facilitating the exchange of health and medical information between clients and healthcare providers.
This role requires effective communication, organizational skills, and attention to detail to ensure seamless transactions and exceptional client experiences.
- Manage client portfolios, ensuring timely actioning of caseloads.
- Liaise with specialists and medical clinics to obtain medical records on behalf of clients.
- Document accurate case notes in our platform.
- Maintain relationships between clients and our organization.
- Perform data entry and administrative duties as required.
- Caseload Management: Manage a portfolio of clients, ensuring caseloads are actioned within specified timeframes.
- Medical Record Retrieval: Liaise with specialists and medical clinics to obtain medical records on behalf of clients.
- Case Note Documentation: Write accurate case notes in our platform.
- Client Relationships: Maintain relationships between clients and our organization.
- Administrative Support: Perform data entry and administrative duties as required.
