About this role

This challenging yet highly rewarding role involves working closely with clients who are experiencing homelessness, providing them with support and guidance to stabilise and sustain their accommodation and help them transition away from homelessness.

Main Responsibilities:

  • Engage with persons sleeping rough and/or those at risk of becoming homeless.
  • Undertake home visits with clients located in temporary, transitional or permanent accommodation.
  • Refer clients to appropriate external services and link to life skill and educational programs.
  • Create collaborative partnerships with other key stakeholders and services.
  • Offer client support at all times with a respectful, non-judgmental and inclusive attitude and manner.
  • Develop case plans with clients, provide ongoing support to achieve the case plan and monitor progress.
  • Maintain accurate records of client interactions and outcomes.

Requirements for Success:

- Previous experience supporting vulnerable individuals.

- Demonstrated knowledge of issues affecting people experiencing homelessness.

- Strong communication and interpersonal skills, ability to build rapport with clients and colleagues.

- Excellent organisational and time management skills.

- Ability to maintain confidentiality and handle sensitive information with discretion.
