Job Description

As a Clinical Lead in Social Work, you will be responsible for guiding specialist services to patients, clients, consumers and their family/carers to ensure optimal health outcomes consistent with policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.

This position is focused on providing a high-quality specialist Social Work service for Homeless Health. It involves assisting people experiencing homelessness to secure and/or maintain stable accommodation through improved care planning and service coordination within health and between health and the broader community sector.

The role will seek to strengthen partnerships and collaboration between health and the community sector, working closely with NGOs responsible for administering brokerage funds. You will lead clinical service improvement across the Illawarra Shoalhaven Health District in the area of Homelessness Health.

Key Responsibilities
  • Providing clinically guided specialist Social Work services to patients, clients, consumers and their family/carers;
  • Ensuring optimal health outcomes through adherence to policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct;
  • Developing and implementing strategies to improve care planning and service coordination for people experiencing homelessness;
  • Strengthening partnerships and collaboration between health and the community sector, including working with NGOs responsible for administering brokerage funds;
  • Leading clinical service improvement across the Illawarra Shoalhaven Health District in the area of Homelessness Health.
