Job Title: Homelessness Health Specialist

The role of the Clinical Lead Social Worker plays a pivotal part in guiding specialist social work services to patients, clients, consumers and their family members ensuring optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation and the NSW Health Code of Conduct.

This position is responsible for delivering high-quality specialist social work service for individuals experiencing homelessness to secure and/or maintain stable accommodation through improved care planning and service coordination within health and between health and the broader community sector.

Key Responsibilities:
  • Clinically guide specialist social work services to patients, clients, consumers and their family members ensuring optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation and the NSW Health Code of Conduct.
  • Provide high-quality specialist social work service for individuals experiencing homelessness to secure and/or maintain stable accommodation through improved care planning and service coordination within health and between health and the broader community sector.
  • Strengthen partnerships and collaboration between health and the community sector working closely with non-government organisations responsible for administering brokerage funds lead clinical service improvement across the Illawarra Shoalhaven Health District in the area of Homelessness Health.

The organisation provides healthcare services to over 210,000 residents across the region. The district includes Shoalhaven Memorial Hospital located on Yuin Country providing emergency care medical surgical and orthopaedic services among others.

Requirements:
  • Applicable vaccination requirements for a Category A role.
  • Working With Children Check WWCC.
  • Social Work degree and eligibility for the Australian Association of Social Workers AASW.
  • Valid NSW Drivers Licence.
