
Homeless Support Advocate

We are seeking individuals to support vulnerable community members in housing and homelessness services.
About the RoleThis role will work collaboratively with colleagues across different programs and sites, providing vital support to clients through various initiatives.
Key Responsibilities:- Assist clients to access home care packages and NDIS by offering referrals and case management assistance.
- Conduct assessments of new residents and develop support plans as necessary.
- Respond to client feedback and complaints in accordance with policy and procedures.
- Help clients maintain lifestyles that promote their health and wellbeing, addressing social isolation.
- Foster engagement and maintenance of natural supports.
- Utilise relationships within sectors to facilitate access to other services and systems.
- Collaborate with support workers and teams to ensure a positive team culture.
- Complete and review client file documentation across program areas for timely and accurate recording.
- A qualification in Social Work, Welfare, Community Services or equivalent is essential.
- A current Victorian driver's license and willingness to travel to various housing sites around Melbourne.
- Experience in a support worker role in Intensive Case Management, specifically in AOD, Mental Health, Family Violence, or Homelessness services.
- Ability to work effectively and sensitively with clients, including those with high support needs and challenging behaviours.
- An understanding of the Housing and Homelessness Service sector, My Aged Care, NDIS, and other relevant service systems.
- Proven commitment to excellence, accountability, and innovation in service delivery.
- Strong communication skills and ability to engage with people from diverse backgrounds.
- First Aid Certificate.
