We are seeking a compassionate and dedicated Case Manager to join our team.
- This is a challenging yet rewarding role that requires empathy, resilience, and a commitment to person-centred, trauma-informed care.
The successful candidate will be responsible for delivering brief interventions, coordinating care plans, and providing assertive outreach to build trust and connect individuals to essential services.
As a key member of our team, you will work collaboratively with colleagues to support people transitioning from chronic homelessness to stable housing through holistic, wrap-around case management.
You will have the opportunity to make a meaningful difference in the lives of vulnerable members of our community.
This role is based in North Melbourne, VIC, and reports to the Greenlight Supportive Housing Team Leader.
Key Responsibilities- Build respectful, trusting relationships with individuals facing trauma and social exclusion
- Provide strength-based, person-centred case management tailored to individual needs
- Coordinate referrals, advocacy and support to help clients achieve housing stability
- Collaborate with a dedicated team to deliver positive lasting outcomes
- Contribute to service innovation and continuous improvement across homelessness programs
- A degree in Social Work, Psychology, Social Sciences or a related field
- Demonstrated experience in case management within the homelessness or community services sector
- A solid understanding of the issues surrounding homelessness, trauma, and complex needs
- A commitment to human rights, social justice, and the values of integrity, compassion, respect, diversity, and collaboration
- Ability to work independently and as part of a collaborative, supportive team
- A current Driver's Licence
- A current Employee Working with Children's Check
